We can generally accept
✓Residents needing help with activities of daily living, including total care
✓Dementia and memory loss, under our DSHS dementia designation
✓Mental health diagnoses, under our DSHS mental-health designation
✓Insulin-dependent diabetes, blood-sugar monitoring, and delegated injections
✓Hoyer lift, sit-to-stand and two-person transfers
✓In-home physician visits available through Seva Medical
✓Hospice-enrolled residents, working with the family’s chosen agency
✓Respite and short-term stays ($275 per day), availability permitting
We cannot accept
·Skilled-nursing or hospital-level medical support
·Ventilator dependency or other specialised equipment beyond AFH scope
·Behaviours that cannot be safely managed in a six-resident household
·Medicaid placements without a suitable open bed, 120 days notice, or below a $125/day CARE-assessed rate
If a referral falls outside our scope we'll say so on the first call, not after a week of paperwork.

We review against our designations and current household mix, and give you a yes, a no, or a question — quickly.

Assessment
A care assessment at the hospital, facility or home, and a tour for the family when they can come.

Placement
Negotiated service agreement, room and rate in writing, and a move-in date confirmed.
